Adrienne Weede
Adrienne Weede, LCSW, is the National VA Transition and Care Management (TCM) Program Manager with Transition and Care Management Services in the Office of Care Management, Chaplain and Social Work, within the Office of Patient Care Services. She is also the daughter and daughter-in-law of an Air Force Veteran, and aunt of a U.S. Marine Corps Pilot. In this position, Ms. Weede provides oversight and program management for the Transition and Care Management Programs located at VA facilities nationwide. Ms. Weede also provides leadership for key initiatives aimed at increasing collaboration and coordination through integrated case management. In this role, she partners with National Program Offices and community partners to align efforts and ensure excellent care and services to our Nation’s Servicemembers and Veterans, and their Caregivers.
